We are delighted you are choosing us for your healthcare needs. We have a couple of options to register you as a new patient. Please feel free to choose any ONE of the below options that is most convenient for you! No matter which method you choose, we encourage you to review our policies on the Clinic Policies Page.
Please also be sure to bring government issued ID and your Insurance Card to your first visit.
Traditional options:
By Phone. Please call 865-234-2347 to schedule an appointment. You may click on Intake.pdf to download, print and fill in the new patient paperwork at your convenience. Simply call for an appointment and bring the first page of the form with you. We also have paper forms available in our clinic if you do not readily have access to a printer.
Preferred option:
Web Form. The below form can be used to pre-fill the basic new patient paperwork. We will contact you by phone within the next business day to schedule an appointment.
