We are delighted you are considering choosing us for your healthcare needs. We have four options to register you as a new patient. Please feel free to choose any ONE of the below three options that is most convenient for you!
Please also be sure to bring government issued ID and your Insurance Card to your first visit.
Traditional options:
By Phone. Please call 865-234-2347 to register by phone and schedule an appointment.
Print paperwork. Click on New Patient.pdf to download, print and fill in the new patient paperwork at your convenience. Simply bring the forms with you on your first visit. We also have paper forms available in our clinic if you do not readily have access to to a printer
Preferred options:
E-Form. The below form can be used to pre-fill the new patient paperwork. We will contact you the next business day via provided phone number to schedule an appointment.
